The official LS Forms that you need for the different LS Academic Procedures
Guidelines
AISIS
- Forms with the AISIS button are ONLY available through your AISIS Account upon Enrollment
- Tap the button, and log-in to your account
- Course Curriculum can be accessed by logging-in, and choosing Official Curriculum
- Print the form, answer (if necessary), and submit to the corresponding offices
get_app
- A highlighted get_app button gives you a PDF copy
- Tap the button to view the file on your browser (make sure that the browser's PDF viewer is enabled to avoid auto-downloading)
- Download and/or send the file to an email
- Print the form, answer (if necessary), and submit to the corresponding offices
edit
- A highlighted edit button gives you an editable Google Doc copy of the file
- Tap the button, and choose "Make a copy" (this will save the copy into your active Google Drive)
- Edit and fill out the form
- Print the form and submit to the corresponding offices
